Name: Hypertension Clinic
Length: 2.5 hours
Description: This clinic provides a team approach to supporting and treating patients with hypertension. The team consists of a nurse, dietician and pharmacist working in consultation with the patient's primary care provider (PCP). The team empowers patients to manage their own hypertension through lifestyle (diet, exercise, stress-management) and pharmacotherapeutic strategies. Individual appointments are 1.75 hours in length during the clinic times, and must be booked in advance.
Learning Objectives:
- To provide education about blood pressure, hypertension and risk factors
- To take 6 BP readings
- To discuss home BP monitoring
- To discuss antihypertensive medications
- To discuss medications to reduce overall risk of cardiovascular disease
- To encourage physical exercise & wt reduction
- To discuss the Dietary Approaches to Stop Hypertension (DASH) diet and reduction in salt intake
- To develop an Action Plan
