Diabetes Nurse Educator/Registered Nurse: Educates patients on diabetes management and prevention, diabetes medications, diabetes complications, blood pressure targets, cholesterol targets, glucose targets and the use of glucose monitors.
Dietitian: Educates patient on diabetes management and prevention, physical activity and healthy nutrition (e.g. fiber, glycemic index, carbohydrate goals, carbohydrate counting, heart healthy diet, low sodium diet, weight control, weight loss, lowering triglycerides) and the use of glucose monitors.
