Name:  Diabetes DIY (Do It Yourself)

Length: 3.0 hours (offered bi-weekly, morning and evening classes available)

Description: This workshop is an interactive session designed to improve your health habits and diabetes self-management.  Each session features a unique collaborative approach to diabetes education that includes interactive education facilitated by a diabetes nurse educator and diabetes dietitian.

Topics - What is Diabetes and how it is diagnosed, risk factors for developing diabetes, preventing complications, targets for optimal blood sugar control, foot care, managing medications, understanding lab results, improving your patient-provider partnership, healthy eating, the Glycemic Index, importance of fiber, physical activity, and goal setting.

Learning Objectives:

  • To learn the basics of diabetes self-management
  • To learn about healthy eating, getting active and setting achievable goals
  • To learn how to maintain healthy changes and plan for the future

Workshop Leads:  Diabetes Dietitian/Diabetes Nurse Educator

Workshop Size: 10

Special Notes: All DEP patients will be required to attend a group session prior to individual counseling with a diabetes educator.

Name:  Craving Change

Length: 4 weeks (2.0 hours once a week)

Description: This workshop is a how-to guide for changing your relationship with food. 

Learning Objectives:

  • To understand WHY you eat the way you do
  • To learn to comfort yourself without food
  • To change your thinking and change your eating

Workshop Lead: FHT Dietitians and Social Workers

Maximum Class Size: 10

Special Notes: Participants must be 16 years or older and patient of TC FHT. This workshop does not support individuals suffering from eating disorders. These conditions are best managed with individual nutritional counseling.
