Diabetes Education Program

The goal of this program is to give people living with diabetes, or at risk of developing diabetes, the knowledge
and skills needed to become self-managers of their diabetes.  A significant focus is on the prevention of
complications by achieving optimal glycemic control. Services include individual one-on-one counseling & group education.

Patients living with diabetes or at risk of developing diabetes would benefit from a primary care referral to the Diabetes Education Program.  Its primary purpose is to provide basic education on how to prevent or how to mange diabetes on a daily basis.  The Diabetes Education Program sees patients quarterly helping them learn how to live well with diabetes and how to prevent complications.
