<p><span style="font-family:Arial;"><span>Victoria Paulionis has been hired as the Registered Nurse for S522 (replacing Rebekah Barrett) starting Apr 22, 2013.  </span></span><span><span style="font-family:Calibri, Verdana, Helvetica, Arial;">Victoria earned her Bachelors of Science in Nursing in 2010 and her Bachelor of Science in 2007 both from UofT.  Victoria comes to us with 3+ years of primary care nursing experience having worked with Marlee Medical Center, Summerville and Women's College FHTs. </span></span></p>]]></description>

				<description><![CDATA[Taddle Creek FHT Launches Well Child Care GroupOn November 5, 2012, Taddle Creek FHT held its first Well Child Care Group for 6 fourth month old babies.  This is an innovative model of well child care that brings children of the same age together for their routine 'check ups' including growth measurements, hands on examinations, and immunizations.  The visits involve various members of the team (e.g. nurses, nurse practitioners, physicians, dietitians, social workers, pharmacists) who cover many key, relevant topics (e.g. developmental milestones, teething, introducing solids, teething, sleep, behavior and family issues, etc.).  Group visits are held at 4, 6, 9, 12 &amp; 15 months and individual visits with the primary care provider are at 2 and 18 months.]]></description>

				<description><![CDATA[Ontario's Action Plan to Transform Health CareOntario recently launched an Action Plan to transform the health care system and deal with the demographic challenges as well as the province's deficit (see attached announcement).  The Action Plan will ensure families get the best health care where and when they need it, while ensuring all Ontarians get better value for their health dollars.  The Plan speaks to giving Ontarians better access to family doctors and nurse practitioners - through after hours care and same-day and next-day appointments.  As a result of this announcement, TC FHT was approached by CBC for a televised interview and an interview for their Ontario Today radio show.  Dr. Pariser did the televised interview on Jan 30 and the radio interview Jan 31.  The link to the radio interview is below if you would like to listen to it.  ]]></description>

				<description><![CDATA[Taddle Creek FHT Raises Close to $1000 for Alzheimer'sOn Saturday, January 29, 2011, Taddle Creek FHT joined over 1500 walkers in the 2011 Manulife Walk for Memories, supporting the Alzheimer Society. The members of team "Taddle Tails", whose lives have each been touched by Alzheimer's in some way, was able to raise close to $1000.  The Alzheimer Society raised more than their goal of $500,000 through this inspirational indoor walk. <p>Taddle Creek Family Health Team hosted a visit from the Ontario Ministry of Health and Long-Term Care, the Honourable Deb Matthews, on Nov. 2, 2010. Minister Matthews announced that 5 million Ontarians are now receiving the benefits of electronic medical records. The benefits of electronic medical records are many, especially enhancing communication amongst healthcare professionals, facilitating medication management and access to patient information at point of care.</p>]]></description>
