Victoria Paulionis has been hired as the Registered Nurse for S522 (replacing Rebekah Barrett) starting Apr 22, 2013. Victoria earned her Bachelors of Science in Nursing in 2010 and her Bachelor of Science in 2007 both from UofT. Victoria comes to us with 3+ years of primary care nursing experience having worked with Marlee Medical Center, Summerville and Women's College FHTs.
Taddle Creek FHT employees win AFHTO Bright Lights Awards
At the Association of Family Health Team Organization’s October 2012 Conference, two Taddle Creek FHT employees won Bright Lights Awards. One for Best Practices in Health Promotion and Chronic Care, won by Megan Omstead, and the other for Strengthening the FHT Team, won by Sherry Kennedy. Congratulations.

Minister of Health Deb Mathews visits Taddle Creek FHT Announcing Health Links
On Dec 6, 2012, the Honorable Deb Matthews, Minister of Health, visited Taddle Creek FHT to announce the creation of 19 Health Links across the province. Health Links will improve the co-ordination of care for high-needs patients such as seniors and people with complex conditions. Taddle Creek FHT is coordinating the Mid-West Toronto Health Link encompassing a geographic area west of Yonge St., east of Dovercourt, South of Eglinton and north of Lake Ontario. We are very excited to be working closely with our community partners, hospitals, other FHTs, CCAC, Toronto Public Health and the Toronto Central Local Health Integration Network (LHIN) on this project.

Taddle Creek FHT Launches Well Child Care Group
On November 5, 2012, Taddle Creek FHT held its first Well Child Care Group for 6 fourth month old babies. This is an innovative model of well child care that brings children of the same age together for their routine 'check ups' including growth measurements, hands on examinations, and immunizations. The visits involve various members of the team (e.g. nurses, nurse practitioners, physicians, dietitians, social workers, pharmacists) who cover many key, relevant topics (e.g. developmental milestones, teething, introducing solids, teething, sleep, behavior and family issues, etc.). Group visits are held at 4, 6, 9, 12 & 15 months and individual visits with the primary care provider are at 2 and 18 months.
Taddle Creek Keeps on Growing
We recently hired two new staff members here at Taddle Creek Family Health Team. Welcome Brenda Ponic, Lead Social Worker. Brenda earned her Masters of Social Work from McGill University in 1997 and she also holds a BA in Sociology & Women’s Studies. Her professional experience includes being a SW for 9 years at the Women’s Health Center at St. Joseph’s and a psychotherapist at Family Transition Place in Orangeville.
Also welcome Shawn Goodman, Pharmacist. He has been hired for one day a week. Shawn earned his Doctor of Pharmacy from the University of Florida in 2012. He also holds a Bachelor of Science in Pharmacy and a Bachelor of Science in Biology. His professional experience include being a pharmacist at Etobicoke & Caroline Family Health Teams, 1 year of hospital experience at Shands Hospital in Gainesville, Florida. In addition, Shawn has years of experience at private pharmacies.

Victoria O'Neill has been hired for the Bloor Site Nurse Practitioner position, she will be working primarily out of Suite B102.
Victoria earned her Masters of Science/Nurse Practitioner degree from D'Youville College in Buffalo, NY. Victoria also has her Bachelor of Science in Nursing from Texas Woman's University and a Diploma of Nursing from George Brown College, Toronto. Her professional experience include being a registered nurse at St. Michael's Hospital for 6 years in the cardiovascular intensive care unit, a clinical instructor at Humber College and NP locums at local Community Health Centers.
Victoria's 1st day will be Jan 3, 2012.

New Staff Hired
Aislinn Hemingway has been hired for the Bloor Site Registered Nurse position, working primarily out of Suite 207. Her 1st day will be Jan 3, 2012. Aislinn earned her Masters of Nursing (Leadership in Education & Health Care Policy) from Ryerson University. Aislinn also has her Bachelor of Science in Nursing from UofT. Her professional experience include being a Research Officer with the Faculty of Nursing, University of Toronto, an Advanced Practice Nurse Educator in the Burns, Plastic Surgery & Ophthalmology Units at the HSC and a RN in Cardiology at the HSC.
Shivon Sanduja has been hired for the 2nd Diabetes Dietitian position (in Mar 2011, TC FHT was approved a 2nd Diabetes Education Team increasing our compliment to 2 Diabetes Nurse Educators, 2 Dietitians and one FT Admin. Ass’t). Her 1st day will be Jan 3, 2012. Shivon has a BSc. from Western & a B.A. In Health Studies from McMaster. She is a graduate of UHN’s Accredited Dietetic Internship Program. Her professional experience include working as a Registered Dietitian at UHN for 1.5 years and as a Diet Clerk for 2.5 yrs at the CAMH.

We would like to welcome Lauren Cripps, Registered Nurse, here at Taddle Creek Family Health Team. She will be here temporarily while another nurse is away on leave. She has 2 years of experience working at The Hospital for Sick Children and as Research Officer/Lab Instructor at U of T. Lauren has her Bachelor of Science, Nursing from the University of Toronto and is working towards her Masters of Nursing.

New Social Work Student here at Taddle Creek
We are happy to welcome Andrew McKenzie, U of T MSW student here at Taddle Creek Family Health Team. He will be starting in September, and work as a part of the Mental Health Team each week from Wednesday to Friday until next April.
What's Happening at Taddle Creek
Over the summer we've had a few changes to our staff. Our Diabetes Nurse Educator, Jessica Bakhle, had her first baby! Lila was born on Aug. 13 and weighed 7lbs, 9oz. Lila's arrival prompted us to hire a well qualified replacement, Ashen Chetty, who has 13 years experience working with diabetes.
After a careful search, we have also found a replacement for the nurse at our Bloor site (Cailtin). In July, Taraneh Pourdowlat joined our team. She brings a wealth of experience from her years of specializing in maternity, labour and delivery.

A New Social Worker at Taddle Creek FHT
TC FHT has hired Raymond Chong for the vacant social worker position. Raymond started with us on Apr 27, 2011 and is located at 790 Bay, Suite 306.
Raymond graduated from the Master of Social Work program at the University of Toronto in Apr 2009. He specialized in gerontological social work and has taken courses to give him valuable knowledge on issues facing the immigrant communities. His work experience includes the role of Psycho-geriatric Case Manager at Community Care East York (2 years) and Liaison Worker at Parkdale Community Information Center where he counseled newcomers.

Taddle Creek FHT Raises Close to $1000 for Alzheimer's
On Saturday, January 29, 2011, Taddle Creek FHT joined over 1500 walkers in the 2011 Manulife Walk for Memories, supporting the Alzheimer Society. The members of team "Taddle Tails", whose lives have each been touched by Alzheimer's in some way, was able to raise close to $1000. The Alzheimer Society raised more than their goal of $500,000 through this inspirational indoor walk. The Daily Bread Food Bank raised over $1 million and 1 million pounds of food with their 2010 Holiday Food Drive. Taddle Creek FHT was proud to participate in this Food Drive - members of the team generously donated non-perishable goods and over $700 for this worthwhile cause. G20 Summit and Taddle Creek
Due to the close vicinity of the Bay street location to the G20 protest sites, the Bay location of Taddle Creek will be closed on Friday June 25, 2010.

New Registered Nurse At Taddle Creek
Taddle Creek Family Health Team welcomes Caitlin Houselander, Bloor Site Registered Nurse (replacing Jessica Ross).
Caitlin earned her Bachelor of Science in Nursing from the University of Western Ontario in 2009. Her professional experience includes both public health (at the Region of Peel and Wellington Dufferin) and primary care nursing (at the Byron Family Medical Clinic in London).
Caitlin’s 1st day will be May 31, 2010.
