Diabetes Nurse Educator [Job Description]

We are looking for a Diabetes Nurse Educator to join an established FHT that has received dedicated Diabetes Education Program (DEP) funding. Fourteen primary care providers as well as community physicians refer patients to the DEP. This FHT has dedicated DEP space/equipment to teach patients chronic disease management (diabetes).
