Primary Care Providers: Perform health assessments, diagnosis, treatment and supportive mental health/palliative care.
Note: At this time only two primary care providers perform primary care home visits.
Note: Primary Care Providers will engage other interdisciplinary health providers (e.g. social worker, nurses, dietitians) on an as needed basis.
For example, the primary care provider may ask a social worker to accompany them on the visit in order to connect the patient with community resources (e.g. Community Care Access Center) or to provide support to family caregivers. A second example might involve a registered nurse attending the visit to offer chronic disease management education.
